SLCC student 1098-T forms lost after memory device disappeared in the mail

SALT LAKE CITY, Utah (ABC4 News) – Salt Lake Community College is reporting the loss of student 1098-T tax forms.

SLCC uses a vendor to process, print and mail 1098-T forms to students. This process is done through a secure portal between the college and the vendor. The vendor also sends a memory device via email with a PDF of each form.

During the last transportation to the college, the envelope was damaged, and the device was lost. The forms contained addresses, total tuition, fees paid and the last four digits of social security numbers. 

Given the type of information on the device, SLCC does not believe the situation to be a credible threat to identity security. Officials say it is very unlikely the content on the device was compromised.
