Out-of-state driver killed in head-on collision near Duchesne

 DUCHESNE, Utah (News4Utah)- An out-of-state driver was killed in a head-on collision Thursday night.

It happened around 11 p.m. just outside of Duchesne on SR-40 near mile marker 80. Troopers say the driver was heading westbound and suddenly crossed into the eastbound lanes, colliding with a semi-truck pulling double tankers.

The tractor caught fire after the collision, but troopers say the driver was not badly injured.

Investigators say the driver of the car died on impact. He has been identified as Jacob A. Kosowsky, age 21, out of Boca-Raton, Florida. Troopers say the passenger was transported to the hospital with non-life threatening injuries.

The exact cause of the crash is still being investigated.
