Mortuary allowed to re-open under certain stipulations

SOUTH SALT LAKE (ABC4 Utah)-A mortuary whose license was suspended by the state after accusations of misconduct will be allowed to re-open, but only under certain stipulations.

Earlier this month, Carver Mortuary was closed after allegations of unethical and unsanitary practices. Court documents say that between the months of Feb. and Nov. this year the funeral service knowingly:

  • Mixed fetal and infant remains and placed them in the same retorts as adults.
  • Hired employees who were not licensed, allowing them to perform embalming and cremation services.
  •  Cremated people without the proper paperwork.
  • Failed to properly clean the ashes left behind after each cremation.
  • Left unembalmed bodies outside of the refrigerator for more than 1 day.

Tuesday, it was decided the mortuary can operate on probation for five years if they agree to follow certain terms. These terms include hiring a full-time licensed funeral services director and operation supervisor to oversee the workflow and reporting to the Division and Board weekly.

They must also submit a practice plan that implements new policies and procedures. Under the Stipulation Order, the Division has can appear on facilities at any point and inspect the property.

The order also says if an owner or supervisor employee is convicted of a criminal offense the Division may revoke or suspend their license.

Carver Mortuary must also pay a $10,000 fine. If any violations are found at any point they will have to pay $50,000.
