PROVO, Utah (ABC 4 News) - A divorce decree obtained by ABC 4 News shows that Gary Coleman was divorced from the woman who made life and death decisions on his behalf.

Coleman was removed from life support and died last Friday.  He had received a head injury Wednesday after falling at his home in Santaquin. According to police, Coleman was weakened by a dialysis treatment that day and probably fell because of that.

During a 911 call, Shannon Price identified herself as Gary Coleman's wife.

Coleman was rushed to Intermountain Health Care's Utah Valley Medical Center in Provo, where he eventually slipped into a coma. Hospital officials identified the cause of death as "intracranial hemorrhage".

The divorce decree was signed in August of 2008.

Coleman's attorney Randy Kester told ABC 4 News the divorce was "amicable."  It was initiated by Coleman.  He said Shannon Price decided not to be represented by an attorney during the process.

The decree was filed under the names John and Jane Doe because the parties asked that documents related to the divorce be sealed.
